Project Consent Form - New in 2012 - please use this form for all sport
programs associated with the Peninsula School Sport Program (including
district teams) and return them to your school for storage. This form will
remain current for all representative school sport for the duration of your
child's enrolment at their present school (ie. if you change schools you will
be required to complete a new form).

Request Form - needed by manager 1 week after selection. This replaces
private accommodation form and is also used if you are requesting a billet.
